Job Summary

Licensed Clinical Social Worker: UCDH Home Care Services Licensed Clinical Social Worker (Weekend Social Worker) services are provided to individuals and caregivers in their place of residence for the purpose of promoting, maintaining or restoring health, or minimizing the effects of illness and disability. The Weekend Social Worker provides comprehensive clinical social services that are appropriate to the needs of the individual patient, family, and caregivers, and functions as an integral member of the interdisciplinary team..


Social Worker 2: Home Care Clinical Social Work services are provided to individuals and families in their place of residence for the purpose of promoting, maintaining or restoring health, or minimizing the effects of illness and disability. The Home Care Clinical Social Worker (MSW/LCSW) provides comprehensive clinical social services that are appropriate to the needs of the individual patient, family and caregivers and functions as an integral member of the Interdisciplinary Team. Social workers support patients with conditions that span the entire health continuum including end of life. Home Care Services Social Workers work collaboratively and in concert with physicians, nurses, and other allied health professionals. Clinical Social Workers assist patients and their families by addressing and resolving the social, financial and psychological problems related to their health condition and facilitate patient movement through the health continuum. The primary responsibilities for this position consist of providing a full range of evidence-based social work services to patients in Home Health, Hospice, HEAP and/or Home-Based Palliative Care. Social work interventions are patient-centered and guided by the patient's cognitive, psychological, and emotional functioning, communication and learning capabilities, physical abilities, support system, living environment, and social determinants of health. Social work services include, but are not limited to, the following: biopsychosocial assessments, crisis interventions, suicide risk and safety planning, supportive counseling, grief counseling, abuse/neglect mandated reporting, symptom reduction, harm reduction, education and support to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation, and facilitating linkage to vital community-based resources for patients and families who are experiencing significant emotional, social, environmental, or financial stress due to hospitalization, acute, chronic, or terminal illness and/or who need help in meeting their continuing care needs. The Clinical Social Worker must adhere to policies and procedures and adapt easily to changes to workflow, work standards, work practices and work assignments as a result of advancements in technology, healthcare coverage changes, new/revised regulatory requirements, and new social work practice standards and programs. The Clinical Social Worker may be asked to participate in special projects and collect data to improve the quality of services delivered to patients. The supervisor and manager have total responsibility for work assigned, work schedules, performance evaluations, corrective action, promotion and dismissal.

Standard shifts are ten (10) hours, three days per week. Weekend availability is required, with potential for one weekend off each month. Work environment may include clients' homes, hospitals, care facilities, and clinics. Must be able to travel independently to these locations, using personal vehicle, at various times of day, including home settings with possible unhygienic conditions and/or homes with indoor/outdoor pets. Travel distance may vary based on client location. Must participate in the DMV pull notice system. Auto insurance must comply with UCDH Travel Regulations Policy G-28, pg. 18, item iii; Insurance Coverage: 50/100/50. Must be able to work occasional evenings, overtime, on-call, weekends, and holidays as defined by the collective bargaining agreements (if applicable).

Minimum Qualifications

  • Licensed Clinical Social Worker
    • California Licensed Clinical Social Worker (LCSW)
    • Valid California Board of Behavioral Sciences Licensed Clinical Social Worker license without restrictions.
    • American Heart Association (AHA) Basic Life Support (BLS) or UC Davis Health CPR
    • Valid California driver's license without restrictions
    • Required to hold a valid California driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program
    • Master's Degree (MSW) in social work from an accredited school of social work.
    • Experience within an acute medical hospital, mental health setting, and/or health care clinic.
    • Basic computer skills.
    • Ability to read, write, speak, and understand the English language. Excellent written and verbal communications skills.
    • Ability to utilize medical terminology appropriately.
    • Ability to remain calm under pressure.
    • Commitment to excellence in patient care
    • Demonstrates accountability and professionalism.
    • Ability to organize a variety of concurrent activities, set realistic goals and objectives, and plan/meet deadlines without compromising quality.

    • Knowledge of legal implications in clinical practice and documentation.
    • Knowledge and understanding of individual, child, and family development, human behavior and communication, the influence of culture and values in social work practice, psychotherapy, case management, and the relationship between social work practice and medicine.
    • Knowledge of child abuse, elder abuse, and domestic violence reporting requirements, and other significant legislation and laws regulating social work practice, e.g., Tarasoff, patient confidentiality, etc. Ability to make appropriate interventions based on law and professional ethics.
    • Ability to provide consultation, interpret clinical findings and diagnosis, make recommendations regarding treatment, care, and disposition.
    • Ability to use clinical supervision to enhance professional growth and examine clinical work.
    • Ability to work independently with minimal supervision. Strong critical thinking and problem-solving skills to manage multiple levels of information and responsibilities, and to quickly assess complex problems to develop potential solutions.
    • Skills, knowledge, and ability to complete psychosocial assessments for pediatric/adult/geriatric populations.
    • Knowledge of suicidal behavior, the ability to assess lethality, and the ability to develop appropriate safety plans with the patient and to provide recommendations for follow-up care to the patient and interdisciplinary team.
    • Ability to assess adjustment to illness and coping, as well as provide brief counseling for individual and caregivers.
    • Skills, knowledge, and ability working with death/dying/bereavement issues.

  • Social Worker 2
    • Valid CA driver's license
    • Masters Degree in Social Work from an accredited School of Social Work. Home Health and/or hospice experience.
    • Knowledge of and skills to develop and maintain team effectiveness and cohesion and to establish and maintain a work environment based on mutual respect. Knowledge and understanding of human behavior, psychotherapy, communication, the influence of culture and values, and social work practice. Knowledge of suicidal behavior, the ability to assess lethality, and the ability to develop appropriate treatment plan. Ability to set priorities and organize work. Ability to make independent clinical decisions in routine patient care matters. Ability to work independently but may require supervision in complex situations. Ability to work accurately and quickly under pressure. Ability to maintain confidentiality of patient information. Knowledge of community resources and mandated reporting requirements for CPS, APS and domestic violence. Ability to identify one's own professional strengths and weaknesses and to engage in activities designed to enhance effectiveness and to make constructive use of feedback from peers, subordinates and supervisors. Skills to perform CPR and participate in advanced care planning discussions. Knowledge of third-party reimbursement systems, procedures and Federal and State regulations pertaining to Home Care/Hospice. Knowledge of legal implications for clinical practice. Basic knowledge of performance improvement and research process. Ability to represent the University, Department and Home Health/Hospice program in a professional manner. Knowledge of medical terminology.
